HOW TO ENROLL AS A PATIENT WITH STAR
To determine if you are eligible to be a Star patient, please perform the following:
- Verify you have an HMO health insurance plan.
- Verify if your HMO health plan is one of our contracted health plans.
- Select your Star Medical Associates IPA as your medical group and select your primary care physician (PCP) within Star Medical Associate’s network.
